Insurance and Fees

LCCC accepts and is “In Network” with the following plans:

• Aetna Behavioral Health
• Anthem Blue Cross Blue Shield/Anthem Health Keepers
• Cigna Behavioral Health
• Optum Behavioral Health/United Health Care

Coverage must be confirmed, copays and fees collected before the start of services.

If LCCC does not accept your plan and is considered an “out-of-network” provider.

• We do not submit insurance claims for our clients. 
• We can provide monthly statements to clients to submit to their health care insurance provider for out-of-network reimbursement according to the provisions in their plan. 
• By submitting the reimbursement forms to insurance companies, clients can significantly reduce their overall out-of-pocket costs.
• Please consult your insurance company for reimbursement policy information.

Note: Insurance companies require a DSM-V diagnosis for behavioral health services. 

Fees for Diagnostic Evaluations and Therapy Services

Licensed Therapist

 Fees are determined based on service and session length. Please contact the office today to discuss your options.

• Behavioral Health Evaluation: (Individual – $150.00) (Couple and Family – $175.00).
• Outpatient Session: (Individual – $75.00 – $125.00 per session) (Couple and Family – $150.00 per session).
• Group Counseling: $50.00 per session.
• Substance Abuse Evaluation $200.00.
• Substance Abuse Outpatient (Individual – $125.00 per session)

Fees for Diagnostic Evaluations and Therapy Services

Licensed Resident in Counseling (Pending Licensure & Supervised by Licensed Therapist)

 Fees are determined based on service and session length. Please contact the office today to discuss your options.

• Behavioral Health Evaluation: (Individual – $100.00) (Couple and Family – $125.00).
• Outpatient Session: (Individual – $50.00 – $100.00 per session) (Couple and Family – $125.00 per session).
• Group Counseling: $40.00 per session.
